Derry was removed on a stretcher after an awful clash of heads with Nottingham Forest defender Zach Abbott on his debut.
Awoniyi sealed the result in the 52nd minute, completing his brace to put the visitors firmly in control.
Chelsea pulled one back late on through Joao Pedro, who scored a stunning consolation goal in extra time with a scissors kick to reduce the deficit.
Despite dominating possession, Chelsea failed to convert their chances, with sections of the home crowd leaving Stamford Bridge early as frustration grew.
The defeat extends the Blues’ poor run to six matches without a win, including losses to Brighton, Manchester United, Manchester City, Everton, and a draw to Fulham.
